Program launched to help prevent unnecessary hospital readmission

BIRMINGHAM, Ala. (WIAT) — A new program launched by the Baptist Health System aims to educate patients and encourage patients for self-care.

The Baptist Health System Calls Program is a nine-month pilot launched Tuesday.

The program also helps prevent unnecessary readmissions to the hospital.

It’s available to patients who leave near Princeton Baptist Medical Center.

Those who want to participate will need to be referred by a case manager.
